few simple steps. CDP

If you don't have any agreements or documents from your cable, satellite or phone company to submit, you may go to the Contact Us page and complete the Information Request Form and click on "Submit."
2. If you do have agreements or documents, download and print the information form which
includes a description of the
property you wish CDP to
review.

Submit a copy of any agreements
you have with providers of video,
high-speed data and telephone
service to CDP. Also, if
possible, include copies of
revenue share checks you may have
received from the provider.

Complete the form and together
with any and all agreements and
copies of checks received from
the providers, mail

Following this procedure, your information will go directly to the experienced professionals at CDP and they will respond quickly to your questions. CDP will review all documents and make appropriate recommendations for further actions, which includes providing agreements for you to review with your attorney/representative, and an outline of the steps that will follow.
